AUTHORIZATION TO RELEASE criminal history information, prior employers dishonesty, drug offence or violence information, credit bureau reports or motor vehicle reports. In consideration of employment, I hereby authorize Highway Safety Devices (HSD) to make inquiries from consumer reporting agencies concerning my suitability and qualifications including (i) any public record of any conviction for crimes of violence or dishonesty, (ii) any incidents of employment dishonesty, theft, or other employment related acts of dishonesty, violence or drug related offences, or (iii) any credit bureau reports. I further authorize any government agency where such conviction information is on file or any prior employer where such incident occurred to release such reports to HSD. During any period while I am employed by HSD, I hereby authorize HSD to make further like inquiries as may be necessary for employment purposes. I also authorize any such government agencies, such as credit bureau’s and/or any such prior employer to issue such reports in response to HSD’s inquiries. I waive any further notice with respect to prior employers or with respect to such government agencies of credit bureau’s dissemination of any such reports. I hereby generally release and fully discharge HSD, any such government agency, any such credit bureau and any such prior employer from and against any and all liability with respect to or arising from the release or dissemination of any such information for such purposes. I understand and agree that my employment, promotion or retention may be determined in whole or in part based on the reports issued to HSD. I have been informed and I understand that I may obtain a copy of any such report and that I may dispute the accuracy or completeness of the information reported to HSD by contacting any such government agency, credit bureau or prior employer directly.
