EMPLOYEE BENEFITS SUMMARY


• Health Insurance
Group Health Insurance is available through AvMed Health Plans for all
eligible employees via weekly payroll deductions. There are two options available: an Open Access HMO plan and an Open Access POS plan. Highway Safety Devices pays $250-$300 per month towards the premium for every employee enrolled in either plan. This results in an annual benefit of $3,000-$3,600 to each participating employee.
Employees are eligible for enrollment upon the 91st day of continuous
employment with Highway Safety Devices.


• 401K Retirement Plan
Highway Safety Devices strongly encourages its eligible employees to
participate in the company sponsored 401K Retirement Savings Plan.
Highway Safety Devices will match employee contributions, dollar for
dollar, up to 6% of their annual salary. Employees are eligible for
enrollment in the retirement savings plan upon one year of continuous
employment with Highway Safety Devices. (Employee must be at least 18 years of age to enroll.)


• Life Insurance
Highway Safety Devices offers a $20,000 employer paid life
insurance
policy for all eligible employees through Prudential. Enrolled
employees also have the option to purchase additional individual and
dependent life insurance if desired via weekly payroll deductions.
Employees are eligible for enrollment upon the 91st day of continuous
employment with Highway Safety Devices.


• Dental Insurance
Dental Insurance is available through CompBenefits for all eligible
employees via weekly payroll deductions. There are two dental plan
options available: a Dental HMO plan and a Dental PPO plan. Employees are eligible for enrollment upon the 91st day of continuous employment with Highway Safety Devices.

• Vision Insurance
Vision Insurance is available for all eligible employees through
CompBenefits via weekly payroll deductions. Employees are eligible for
enrollment upon the 91st day of continuous employment with Highway
Safety Devices.


• AFLAC Individual Benefits
Traditional medical insurance may not cover every expense related to an illness or injury and this is why Highway Safety Devices offers its eligible employees additional voluntary insurance benefits through AFLAC. Employees are eligible for enrollment upon the 91st day of continuous employment and payment is conveniently made via weekly payroll deductions. The additional voluntary insurance benefits offered include Disability, Accident and Cancer insurance.


• Paid Holidays
Employees are eligible for paid holidays upon the 91st day of continuous employment. The company’s paid holidays are as follows:


New Years Day
Memorial Day
Independence Day
Labor Day
Thanksgiving Day
Friday after Thanksgiving
Christmas Day


• Paid Vacation
During the first two years of employment employees can earn up to five days of paid vacation and as of the third year employees are eligible for ten days paid vacation. Employees are encouraged to refer to chapter two of the Highway Safety Devices Employee Manual for a detailed accrual schedule.


• Paid Personal Days
As of your sixth year anniversary date of continuous employment with
Highway Safety Devices, you will accrue one paid personal day off and
continue to accrue one additional paid personal day off for each year
thereafter with a cap of five days on your tenth anniversary date.


• Bonus Compensation
Employees may be eligible to receive bonuses during the year depending on the performance of the individual and the profitability of the company.

• Referral Bonus Program
Highway Safety Devices offers a $200 referral bonus to eligible employees for the referral and hire of a new employee provided both employees are still employed with Highway Safety Devices at the end of the initial 90-day introductory period. Supervisor eligibility restrictions do apply.


• Payroll Direct Deposit
HSD offers our employees the benefit of direct deposit of payroll checks. This allows for a convenient and hassle-free way to receive your paycheck each week. With your authorization your paycheck will be automatically deposited directly into your bank account which relieves you from having to arrange time to personally pick up your paycheck from the HSD office. Deposits can be made into more than one bank account if requested. You will continue to receive a paycheck stub each week reflecting the total number of hours paid as well as any taxes and/or voluntary deductions made for the week. HSD encourages our employees to take advantage of this time saving and convenient employee benefit.
